Operation & Surgery

Before FUE Reception Technique

Online Consultation:

  1. Regarding online consultation with your photos, you can send us your photos. In order for us to evaluate the hair loss pattern and donor areas correctly, you should send us all parts of your head area with a few clear images.
  2. Your age and origin (race affects the hair characteristics of the donor area).
  3. You should briefly describe your medical history. We need to know about the operations you have had in the past. At the same time, indicate if you have any ongoing treatment.
  4. Indicate whether you use any blood thinners.
  5. Please indicate if you have had a hair transplant operation before.

Preoperative Consultation:

  1. The onset time of hair loss,
  2. Whether there has been any change in your hair loss in the last few months or years,
  3. Your past history with previous medical or surgical treatments and artificial hair systems for your hair loss,
  4. Any known problems or complications caused by your hair loss,
  5. Complete medical history and information on known allergies (if applicable). List of medicines, vitamins or herbal products you are currently taking (if any),
  6. Your family history of hair loss, if any, from both maternal and paternal lineages,
  7. Your requests and expectations regarding the operation,
  8. During the pre-operative consultation, our hair transplant specialist will determine:
  9. Type and stage of hair loss (partial thinning or baldness).
  10. The thickness and type of your hair (wavy, straight or curly).
  11. Hair density in your donor area and also our hair transplant specialist will plan your hair transplant intervention after consultation. Then he will design your new hairline according to your wishes, age and aesthetic stance in terms of appearance.

Pre-FUE Rules:

One week before Fue, please do not take any blood thinners that can increase bleeding (such as aspirin and herbal remedies).

You can live your daily life as usual until the day of the operation.

You can take a shower before or on the morning of the operation day.

You can have a light breakfast or a light meal before the surgery.

Please do not cut your hair short in the last few days just before your intervention. Do not cut your hair as much as possible so that we can evaluate your hair loss and plan the surgery better. Our team will make a haircut for you just before starting the operation.

We recommend that you wear comfortable clothes on the day of the operation.

FUE hair transplantation is a very simple procedure performed with local anesthesia, no special examination or blood test is required, so none will be done.

Planning the FUE Operation

Preoperative Planning:

Micromotor assisted FUE hair transplantation starts with the treatment plan. Our hair transplant specialist evaluates the pattern of hair loss as well as the donor area. After all the details are shared with you, both the reliable donor area and the recipient area are marked and then the hairline is designed.

Marking the donor area to be used for graft extraction:

Hairline design with the help of laser hair pen:

  • The hairline is the strip of hair about 5 mm wide that forms the front of the hair-covered front part. The new hairline should be natural and as dense as possible.
  • According to the patient’s request and the evaluation of the plastic surgeon, the anterior hairline can be designed as V-shaped (triangle), round (oval) or straight. The triangular hairline is suitable for narrower heads and provides natural temporary regression and graft protection for the tops as well. The oval hairline fits well for the wider head and provides a mature and stable temple hair.
  • It is important to plan a suitable hairline according to the age of the patient. With a naturally and genetically high hairline proportional to the patient’s age, 1-2 cm of natural retraction of the hairline should be allowed. The front hairline should not be placed too far.

Estimation of Required Graft Number:

The number of grafts needed for the targeted areas is calculated or if the hair loss is advanced, planning is made according to the number of grafts that can be taken from the patient’s donor area. Rough estimates of the number of grafts needed to transplant all areas of hair loss with good density for each degree of hair loss:

Level 1: 1000 – 2000
Level 2: 2000 – 3000
Level 3: 3000 – 4000
Level 4: 4000 – 4500
Level 5: 5000 – 6000
Level 6: 6000 – 7000
7. Level:  7000 – 8000

Operation Process

  1. Grafts or follicular units (FUs) are synonymous and refer to the basic transplanted units of hair-producing hair follicles.
  2. Depending on the number of grafts to be transplanted, FUE hair transplantation takes approximately 4-7 hours.
  3. You will put on a patient’s gown, then the medical team will shave your head and then begin the surgery.
  4. During the graft extraction (harvest) phase, you will sleep on your stomach or side.
  5. During the canal incisions and implantation phases, you will lie on your back or assume a semi-sitting position. There are break times for meals, refreshments and visits to the toilet. You can watch TV or listen to your favorite music during the intervention.

Local anesthesia application:

The operation begins with the application of local infiltration anesthesia. We routinely use a specific device, the needleless jet gun injector; this is a pneumatic injector that provides a completely painless and comfortable local anesthesia application at the beginning of the surgery.

We use combined short-acting lidocaine and long-acting bupivacaine as anesthetic agents and mix them with epinephrine to prolong the anesthetic effect and also minimize bleeding during surgery. The effect of local anesthesia lasts about 12-14 hours. Surgery and the postoperative period are completely painless and comfortable.

Micromotor assisted FUE hair transplantation is performed in 3 steps:

1-A) Extraction of FUs (follicular units or grafts) from donor sites.

b) Temporary storage and processing/arrangement of Fus in a storage medium.

2-Opening the incisions (channels) in the recipient area (only performed by a hair transplant specialist)

3-Placing (implantation) of Fus into the opened channels (hair loss)

1-A) removal of grafts from the donor area:

Grafts (FUs) are the basic transfer units of hair follicles. FUs are extracted individually from the donor area using small, specially designed extraction punches. The diameter of the tips is from 0.6 to 0.85 mm.

The holes in the donor area are quite small, as a result, there will be minimal scarring in this area and the wounds will heal very quickly.

The resulting grafts are collected using microforceps with very sensitive tips to avoid any tissue damage.

Thanks to the micromotor driven equipment and our highly experienced and skilled graft extraction team members, the extraction of grafts proceeds very quickly and efficiently, reducing the storage time of the grafts and providing complete comfort for our patients.

1-B) Temporary storage of Fus in a cooling storage medium:

  • Next, the grafts are arranged and aligned on surgical pads and temporarily stored in a cool, sterile solution and placed over a Cooling Device that provides a moist environment and prolongs the storage time until implanted in the recipient site.
  • Grafts (FUs) may contain single, double, triple, and sometimes quadruple hair follicles. Accordingly, 1000 grafts (follicular units) can retain and transfer 1500-2500 follicles – meaning the average hair count is 1.5 to 2.5 per graft.

2-Making incisions (holes or channels) at the recipient site for graft implantation.

The incisions are made using very thin custom made blades with diameters smaller than the diameter of the graft extraction needles. The grafts will then completely fill the holes and a secure connection will be established between the grafts and their beds, which is important for the survival of the grafts.

The holes are opened in accordance with the appropriate growth direction of the hair in a certain area of the scalp. Due to this, the natural growth direction and even corner turn is ensured.

We also use Sapphire FUE technique on the front hairline and front sections to provide a smooth and dense hairline.

Opening the incisions standardizes the results of hair transplantation. Thanks to our hair transplant specialist, who has great experience and skills to achieve this, the procedure continues smoothly and efficiently, providing permanent and natural results.

3-Implantation of Fus in pre-made incisions:

Using very small microcracks and with great care and delicacy, the grafts are inserted in the right direction (hair roots inward) into the holes, taking care to place the surface of the grafts slightly higher than the surrounding skin in the transplant area.

Thanks to the expertise and skill of our team members specializing in graft implantations, our goal has always been to achieve 99-100% graft survival rates.

End of operation:

Finally, the final controls are made, the donor and the transplantation area are cleaned with serum. It is covered with bandages and surgical pads to protect the donor area. The cultivation area is left open. The patient is ready to go back to the hotel or home to rest. The postoperative period is completely painless and comfortable.

Hair Growth After FUE

First 3 months (shock shedding, falling and resting phase, no hair growth.)

During the first month, almost all transplanted hair, traumatized by their displacement, will fall out. (“shock loss”). 0-7 days: temporary growth of transplanted hair. 1-4 weeks: hair growth stops and they begin to fall out with crusts.

Next, the follicles enter the telogen resting phase, which lasts 8-10 weeks. Since hair is individual structures; some completely lose contact with the follicles and fall out first, some may retain some fine connections and fall off later, so the transplanted area may look patchy afterwards. Don’t worry.

The hair in your donor area routinely continues in the same way as your hair growth period. Hair that grows in 7-15 days completes your donor area.

3-6 months (new/slow growth stage, ugly duck stage)

After 3-4 months, your new hair starts to grow. First of all, newly growing hair strands may be thin and curly like body hair. The longer it grows, the thicker and straighter the hair becomes.
Due to the fact that your hair grows at different times during this process, the transplanted area may look patchy.

6-9 months (Hair completion phase):

The full lengthening and strengthening of the growing hair is completed.
Your new hair will grow normally (1-2 cm per month) and will continue to thicken/strength over the next few months.

9-12 months (final thickening):

Thickening of the hair (increase in caliber) occurs.
We recommend our patients who have undergone hair transplantation to shave their hair only with scissors for the first year. You can make your haircut only with the help of scissors. Do not use any razors.
The final result occurs 12-15 months after surgery.

Growth rate varies from person to person. Generally speaking, the final result is seen faster in younger patients and in the planting area in the anterior region; On the other hand, it occurs more slowly in elderly patients and in the vertex area.

About a year after the operation, we would appreciate if you schedule an examination to check your results (free of charge). If you cannot visit us, you can send us your photos with your new hair.

FUE donor scar:

In hair transplantation operations performed with the FUE technique, there is no scar after the operation.
It leaves only very small white spots on the donor areas from which the grafts were taken. The appearance of donor scars depends on many factors. (Age, race, skin color etc.)

Post-Operation Haircut

During the first year after the operation, you can shorten or straighten the planted area only with the help of scissors. The first haircut for the transplanted area can begin 1 month after the operation.
You can cut your hair in the donor area at any time (even after 1 week) and use a trimmer, clipper or scissors. After hair transplantation, you are completely free to cut the hair in your donor area.
